Sun Country Health Region is offering a six-week program for community residents of Weyburn and Carlyle this spring. The program, “LiveWell with Chronic Conditions” is a world-renowned program developed by Stanford University.
LiveWell is a self-management program for those people who are living with or caring for someone with a chronic condition. Chronic conditions are long lasting in duration and include conditions like arthritis, diabetes, kidney disease, heart disease, osteoporosis, fibromyalgia, depression, COPD or lung-related illnesses.
This community based program covers topics such as effective communication strategies for loved ones and health professionals, dealing with difficult emotions, goal setting and action plans, medications, nutrition, and physical activity.
The workshops are free of charge and run once a week for six weeks, starting on Tuesday, May 23, in Weyburn and Thursday, May 25, in Carlyle.
Participants each take home a free textbook with information relating to the sessions as well as other helpful information for dealing with chronic conditions.
