Health region offers chronic conditions program

The Sunrise Health Region is offering a program for individuals coping with the everyday challenges of living with a chronic condition titled "Live Well with Chronic Conditions.


The Sunrise Health Region is offering a program for individuals coping with the everyday challenges of living with a chronic condition titled "Live Well with Chronic Conditions."

The sessions are offered to individuals and/or caregivers with chronic health conditions such as chronic pain, cancer, arthritis, diabetes, fibromyalgia, Multiple Sclerosis, heart disease, chronic lung disease, sleep apnea, asthma and Parkinson's disease. Sessions run for six weeks for two hours each and are led by two trained leaders with groups of up to twelve participants.

Participants will learn self-management skills including symptom management, communication skills, relaxation techniques, goal setting, and problem solving.

"The emphasis of the program is on building skills to help manage your heath and maintain an active and fulfilling life," said Cara Murray, Chronic Disease Management Facilitator with the Sunrise Health Region. "The program provides practical suggestions and support, which builds confidence in coping with the everyday challenges experienced by individuals with a chronic condition."

Sessions are offered to anyone with a chronic disease or ers for individuals with a chronic disease. Anyone with a chronic condition who is interested in participating in the "Live Well with Chronic Conditions" program is encouraged to contact the Chronic Disease Management Facilitator with any questions or to register for a session.
